We work with nursing homes and assisted living facilities.
In Home Chronic Care Management Program: This service could be $0 copay. Receive an additional healthcare provider who will not replace your Primary PCP but will be there for month-to- month management for overall health management whether it‘s for assisting with medications, transportation resources, post hospitalization visit, or educating more on current health conditions.
